your establishment's permit ID number with you to
the class, as the Suffolk County Dept. of Health
will need it to issue your
certificate.

miss your class and need to reschedule there is a
$30 rescheduling fee.

Agriculture, this class may not meet their food

Class
Overview
- Class
fee: $150 per person; $110 per person for King's
customers. Starts at 8:30 am, be there at 8
am, ends approximately 5 - 5:30 (you can leave when
finished with exam.) In order to make a reservation,
you must send in payment. Customers can give to
sales consultant, driver, drop off at Kings, mail in,
or pay by credit card. For credit cards, we'll
need your fax number to send you a form to fax back.
Checks should be made out to J. Kings and mailed in
attention Deborah. Payments should include names of
people attending, name of establishment where they
work, and a phone number where they can be reached to
confirm. The certification class is accepted in
Suffolk county only in lieu of going to their class.
It's a one day class, with an exam given the same day.
It takes approximately 2-3 weeks to receive your
certificate if you pass the class. The J. King's Food
Protection Certification Training Class, authorized by
the Suffolk County Health Department, receives
reciprocity (County Certification) in lieu of their
three (3) day course. Upon successful completion
of the Health Department Certification Class and
Exam, you will be mailed a Food Manager's
Certificate from the county. If you fail the exam, you
can come in and take the test again for a fee of $40.
If you are coming in for the test only, you should
arrive at J. Kings no later than 4pm.
- After the class, you will receive a Servsafe certificate upon passing. To get your reciprocal Food Managers Certificate, you will need to send the original Servsafe certificate along with $20 fee to the Suffolk County Department of Health. You will get your Servsafe certificate back with your reciprocal Food Managers Certificate.

previously had taken a ServSafe class at J. Kings, and
need a copy of your certificate, please see below
instructions. Please note: The Suffolk County Health
Department is no longer accepting ServSafe but may give
you the option to take a refresher
class.
How do I order a
duplicate certificate? Duplicate
certificates cost $20 each. To order, submit a
Certificate Request Form:
